Livewell Southwest is a community and mental health services provider in the South West, delivering integrated health and care services across Plymouth, South Hams, and West Devon.

It works in partnership with local NHS organisations to support joined-up, place-based care and improve outcomes for local populations.

What We Did

Livewell Southwest sought to develop new services to address gaps in the local care market and alleviate pressure on acute hospital and community services. This included exploring a short-term care centre and a home-based reablement service. The project focused on defining viable service models, assessing market need, and establishing the financial, operational and commercial foundations required to support successful development and delivery.

What Changed

We collaborated on two inter-connected projects to develop new care solutions: exploring and developing a short-term care centre which offers bed-based reablement support for service users post discharge from hospital trusts. We supported with the development of the business case, including the financial and economic models plus commercial agreements and provided practical programme management support for the service set-up and launch.

Our team supported Livewell to explore their potential to set up and develop a home-based reablement service. This involved extensive market research and development of an innovative service model. Working with the project team to develop a robust service and operating model, as well as building a financial model to ensure that the service remains profitable before going ahead.

Results

The short-term care centre was designed, developed and established within 9 months, providing valuable additional capacity to the health and care system and now serves as a crucial component to support patient flow and improved quality of life outcomes.

The home-based reablement service is currently undergoing governance approval ahead of procurement from the Integrated Care Board.
